Please click on the link below to fill out your forms online. The completed forms will be sent to the school electronically. You will still need to provide the required documentation to complete the process.
Please bring these required documents to the school when registering your child.
- Proof of residence (i.e. utility bill) or an approved Out of Boundary request
- A certified copy of the student’s birth certificate or other reliable proof of the student’s identity and age
- A certified and complete immunization record or official immunization exemption
- Legal Documents (custody, guardianship, court orders, etc.)
