Please click on the registration link to fill out your forms online. All registrations are done electronically. The completed forms will be sent to the school electronically. You will still need to provide the required documentation to complete the process. You may upload them yourself or bring them to the office and we will do that for you.
Please bring these required documents to the school when registering your child.
- Proof of Residence (i.e. utility bill, lease, or mortgage) or an approved Out of Boundary request
- A certified copy of the student’s birth certificate or other reliable proof of the student’s identity and age
- A certified and complete immunization record or official immunization exemption
- Legal Documents (custody, guardianship, court orders, etc.)
Out of Area Students
If your child lives outside of Alpine School District or outside of our school boundaries, please visit Student Services to fill out the proper forms to complete your out-of-area request.
*Please note that filling out the form does NOT guarantee enrollment approval. We highly encourage you to register at your area school to ensure that your child is enrolled.
Free & Reduced Lunch
We encourage families to fill out a free or reduced lunch application. This must be done every year.
- NOTE: Only one application is needed per household.
- The advantage of applying online is that your application is processed within 12 hours. You will receive a letter within 3 to 5 days to let you know if you have been approved. We do not send out emails to notify you.
- Paper applications are available at all school offices and at the Nutrition Services Office 759 E. Pacific Dr., American Fork, UT 84003
